Obtain Request
for Live Scan Form

Your Employer or Requesting Agency will provide you with the Request for Live Scan Service form. Print three (3) copies of the Request for Live Scan Service form.

Make an appointment
with us

Once you have filled out the Request for Live Scan Service form call or e-mail Santa Cruz Live Scan to schedule an appointment

We will come to you and
process your Live Scan

Live Scan is a simple process that takes between 5–10 minutes. Your digital finger prints are securely transmitted to the agency that is doing you background check.

What you will need in order
to process the Live Scan

Completed Live Scan Forms (3). Payment for Service–Cash or CC. Valid Identification–Drivers license (any state), California Identification card, or US Passport (with supporting documentation).
